SLEEKLA Model Agreement
This Model Agreement is entered into as of (Date Below) , by and between SLEEKLA
("Agency"), located at 444 West Ocean Blvd, Long Beach, CA 90802 and
(Name Below) ("Model").
    1. Engagement
    The Agency agrees to engage the Model, and the Model agrees to render services as a model
    under the terms and conditions set forth in this Agreement. The Model agrees to tag
    @sleekmodelsla in their Instagram bio to be considered for bookings.

    2. Term
    This Agreement shall commence on the date hereof and shall continue until terminated by either
    party in accordance with the terms of this Agreement.

    3. Exclusivity
    Model agrees that all bookings and engagements with the BRAND/CLIENT will be made
    exclusively through SLEEKLA. The Model is prohibited from booking any modeling work with
    the CLIENT directly or through other agencies without the prior written consent of SLEEKLA.
    The Model & Agency agrees that this agreement is non-exclusive, and the Model has the ability
    to work with any brands or other agencies.

    4. Services
    The Model agrees to provide professional modeling services at photoshoots, fashion shows,
    promotional events, and other assignments as directed by the Agency. The Model agrees to
    perform such services in a professional manner.

    5. Compensation
    The Agency agrees to compensate the Model for services rendered at rates agreed upon
    between the Agency and the Model. Payment terms will be outlined in individual booking
    confirmations. The Model shall be responsible for the Agency’s Booking fee charged to the
    client for any no call/no shows or cancellations under 24 hours of the bookings.

    6. Conduct
    The Model agrees to maintain a professional demeanor and appearance at all times during
    assignments and to comply with all reasonable instructions from the Agency and clients.

    7. Non-Circumvention
    The Model acknowledges that the Agency has invested substantial time and resources in
    securing clients. The Model agrees not to solicit, negotiate, or accept modeling engagements
    directly from any client introduced to the Model by the Agency for a period of 90 days following
    the termination of this Agreement.

    8. Confidentiality
    The Model agrees to keep confidential all information regarding the Agency's business
    practices, clients, and assignments, and not to disclose such information to any third party
    without the Agency's prior written consent.

    9. Termination
    Either party may terminate this Agreement at any time upon 30 days written notice to the other
    party. Upon termination, the Model agrees to honor all outstanding obligations made prior to
    termination. Model cannot book with clients of SLEEKLA until after 90 days of the expiration
    date.

    10. Independent Contractor
    The Model acknowledges that they are an independent contractor and not an employee of the
    Agency. The Model is responsible for all taxes, insurance, and other obligations as an
    independent contractor.

    11. Dispute Resolution
    Any disputes arising out of or relating to this Agreement shall be resolved through binding
    arbitration in accordance with the rules of the Talent Agency Act (TAA).

    12. Governing Law
    This Agreement shall be governed by and construed in accordance with the laws of the State of
    California.

    13. Entire Agreement
    This Agreement constitutes the entire agreement between the parties and supersedes all prior
    agreements, understandings, and representations, whether written or oral, with respect to the
    subject matter hereof.

    14. Amendments
    This Agreement may only be amended or modified by a written agreement signed by both
    parties.
    IN WITNESS WHEREOF, the parties hereto have executed this Agreement as of the day and
    year first above written.
